To create a new form click on the "ADD" button next to where it says 'Layouts'


This will present you a number of boxes to fill in:


New Layout Name: This is the name of the form, for example "Home Inspection Survey" or "Field Service Report"


There are then upto five index fields which you can specify, these are used for searching for existing forms, and also for creating new ones, so for example for a home inspection survey we might use the following:


Main Index - Postcode

2nd Index - House Number

3rd Index - Street Name

4th Index - District

5th Index - Town


You only need to use 1 index, but it will make it easier for searching if you use more.


Once you have enterred the information click on the create button. The system will then generate the base of the form ready for you to create the sections and questions.
